Will transparent backgrounds stay transparent?

Yes, if “Keep transparency” is on — the PDF page shows white behind it in most viewers but the image itself keeps its alpha channel. Turn it off to flatten to solid white for printing.

Why is my PDF so large?

PNGs of photographs are much bigger than JPGs. Convert the images to JPG first, or run the result through Compress PDF.
